The in-home evaluation is the cornerstone of a detailed care plan. The evaluation can also include a separate conversation with a family member (or conservator or elder-care attorney) to discuss the findings.

 

The personalized care plan provides specific recommendations for addressing the client’s needs — medical, social, and psychological. This action plan includes:

 

1 - Suggestions for steps the family and/or caregiver(s) can implement immediately to help the client minimize confusion and maximize quality of life;

 

2 - Referrals to appropriate medical specialists: neurologist, neuropsychologist, and/or geriatrician;

 

3 - Referrals for in-home care agencies and/or services;

4 - Discuss best plan of care that optimizes independence for person with memory loss.
